This procedure includes counting each piece of sterile equipment that will be used during the procedure. The count must be done by a registered nurse and the sterile nurse or <a href="http://www.artwoo.com/tag/scrub+nurse" rel="tag">scrub nurse</a>. (In some jurisdictions, a scrub nurse can be unlicensed personnel called a scrub technician.) <br /><br /> As each tray of instruments is opened unto the sterile field, the two nurses will compare the contents with a paper sheet which comes with the tray. Each and every piece within the tray will be counted. <br /><br /> All sterile supplies are counted. This includes <a href="http://www.artwoo.com/tag/sponges" rel="tag">sponges</a>, which are usually wrapped together in groups of five or ten. Sponges can be large gauze bundles, or 4"X 8" rectangles of gauze. Also, small gauze pieces which are bound together into a peanut shape and called peanuts are counted. Larger balls of cotton stuffed gauze pillows are counted. Sutures are counted and divided by types. Needles without sutures, or free needles, are counted separately. All of these supplies are possible foreign objects that can be left behind in a <a href="http://www.artwoo.com/tag/body+cavity" rel="tag">body cavity</a>. <br /><br /> There is a debate about which <a href="http://www.artwoo.com/tag/surgeries" rel="tag">surgeries</a> are at risk for lost objects. In most states, the standard is to count instruments, sponges and sutures when a body cavity is opened. A cavity would be the head, abdomen or chest. There is also a risk for left behind sponges in some gynecological surgery where instruments are inserted into the uterus. So sponges and needles are counted for those surgeries. <br /><br /> Major, but minimally invasive surgeries, such as <a href="http://www.artwoo.com/tag/laparoscopic+gastric+bypass" rel="tag">laparoscopic gastric bypass</a>, <a href="http://www.artwoo.com/tag/laparoscopic+nissen" rel="tag">laparoscopic nissen</a> fundalpication, <a href="http://www.artwoo.com/tag/laparoscopic+nephrectomy" rel="tag">laparoscopic nephrectomy</a>, or laparoscopic assisted vaginal hysterectomy are surgeries in which multiple, small puncture wounds are made in which specially designed sheaths are inserted and then the scope and instruments access the cavity through the sheaths. Except for thoracic endoscopic surgeries, utilizing a scope requires the body cavity to be expanded in some way. Laparoscopic surgeries use carbon dioxide gas, arthroscopic, genitourinary and gynecological surgeries use fluid. The sheaths prevent the  gases or fluids from escaping from the cavity. <br /><br /> Technically, it is hard to imagine how anything could be left behind in a surgery done with a scope. However, small screws, jaws to graspers, and parts of staple guns have all been left behind in patients, require additional surgeries and time and money lost. Ultimately it is the responsibility of the surgeon to know if the instrument he pulls out of a patient is complete, but part of the team mentality is that the nurses must also know the construction of an instrument so that if something is missing, it can be accounted for. <br /><br /> An example of this is something that I experienced; I was scrubbed on a back surgery and one of the bone instruments I gave the doctor was missing a screw. It didn't affect it's operation, but I didn't know if it was missing before I gave it to the doctor or not. It required an xray during the surgery to see if the screw was somewhere within the patient's back incision. Many of the cities and towns are experiencing higher than average deaths for cats because there are not enough homes for these pets. The housing and feeding costs are also at an all time high. 2004 showed 1,600 strays were caught and impounded. Of that number less than half were given homes. If a cat owner doesn't want to spay or neuter their pet they must obtain a breeder's license. This license will cost $100. The fee goes towards helping the shelters and humane societies to fund their costs. All you have to do is read about President Garfield's medical care circa 1880 when he was shot by an assassin. <br /><br /> The tale of woe came to our attention as the result of a new exhibit at the <a href="http://www.artwoo.com/tag/national+museum+of+health+and+medicine" rel="tag">National Museum of Health and Medicine</a>, located on the campus of the <a href="http://www.artwoo.com/tag/walter+reed+army+medical+center" rel="tag"><a href="http://www.artwoo.com/tag/walter+reed+army+medical" rel="tag">Walter Reed Army Medical</a> Center</a>. The exhibition is intended to mark the 125th anniversary of his assassination. <br /><br /> To begin with, the gentleman remained alive for eighty days after he was shot twice by an assassin, who will go unnamed, because we don't believe in giving publicity to such <a href="http://www.artwoo.com/tag/miscreants" rel="tag">miscreants</a>, since we think the quick notoriety encourages other deviants. <br /><br /> The first bullet just grazed the President's arm. The second one, however, hit the right side of his back and lodged in his torso. The display shows it penetrated his first lumbar, or lower, vertebra. Today, such a projectile would be easy enough to spot and extract. After receiving a similar shot to the body, President Reagan was up and out of the hospital in no time. <br /><br /> But back then it was an entirely different case. Garfield's doctors, who didn't have modern diagnostic machines to aid their vision, could not decide where the bullet was lodged. A dozen or so medical experts probed the place of penetration, often using unsterilized instruments or bare hands. <br /><br /> Sterile procedures had been developed by then, in the 1860's, by the British surgeon <a href="http://www.artwoo.com/tag/joseph+lister" rel="tag">Joseph Lister</a>, but were not yet appreciated in the colonies. <br /><br /> Historians are in agreement that Garfield died, not of the bullet, but from a massive infection due to unsterile medical practices. <br /><br /> One desperate observer suggested that the docs should just turn the President upside down and see if the bullet would fall out. <br /><br /> The exhibit also includes an image of the metal detector designed by <a href="http://www.artwoo.com/tag/alexander+graham+bell" rel="tag">Alexander Graham Bell</a> to locate the bullet. <a href="http://www.artwoo.com/tag/unfortunately" rel="tag">Unfortunately</a>, his invention failed to detect the location of it. Historians think the reason may be that the device picked up metal coils in the President's mattress or because Mr. Bell only searched on the right side of Garfield's body, where the head physician thought the bullet was lodged. <br /><br /> The autopsy revealed that the fatal shot pierced his vertebra but missed the spinal cord. It had not clipped any major arteries or veins or entered any major organs. It was, in fact, stuck in adipose tissue, that is, in fat, on the left side of his back. <br /><br /> Dr. Ira Rutkow, a professor of surgery at the University of Medicine and Dentistry of New Jersey and also a medical historian, noted, "Garfield had such a nonlethal wound. In today's world, he would have gone home in a matter or two or three days." <br /><br /> In addition to causing the infection with unsterile techniques, Garfield's doctors limited his solid food intake, because they thought the bullet might have punctured his intestines. <br /><br /> "They basically starved him to death," Dr. Rutkow said, pointing out that the President lost over 100 pounds from the time he was shot until he died. <br /><br /> Sterile practice was finally widely adopted in the US in the early 1890's. The X-ray was also discovered in the 1890's. <br /><br /> The assassin was hanged in 1882. Even he blamed Garfield's doctors, stating, "I just shot him." <br /><br /> So, next time you get a big medical bill, just think of Garfield. Canna wild species produced large leaves that were fast growing with a tropical appearance in the landscaped garden. The flowers of wild canna species were small and in gardening circles were viewed as of insignificant notice as a garden subject except for the <a href="http://www.artwoo.com/tag/lush" rel="tag">lush</a> leaves that were highly valued in exotic landscape setting. The flowers were colorful but small and of short duration. The wild canna species were easily inter-crossed to combine widely variable genetic characteristics, until in the year, 1870, Monsieur Crozy inter-crossed an undisclosed number of wild species, that resulted in a celebrated hybrid that he named after his wife, Madame Crozy. <a href="http://www.artwoo.com/tag/luther+burbank" rel="tag">Luther Burbank</a> called this matching of genetic material the beginning of modern canna <a href="http://www.artwoo.com/tag/hybrids" rel="tag">hybrids</a>, after which Burbank and another plant hybridizer, Wilheim <a href="http://www.artwoo.com/tag/pfitzer" rel="tag">Pfitzer</a>, also entered their own charming canna hybrids. <br /><br /> Luther Burbank, the notable American botanist and prolific hybridizer, was well aware of the phenomenon of back crossing various related <a href="http://www.artwoo.com/tag/canna+lilies" rel="tag">canna lilies</a>, and he was determined to achieve a rapid advancement in hybrid vigor by a recombination of desirable and variable plant genetic characteristics. From this large pool of variations in canna lilies, such as color, size, vigor, insect and disease resistance and cold hardiness, the plant breeder could select outstanding canna flower and leaf hybrids to market as newly named cultivars in the world of horticulture. The fact that a cross between two wild species from different continents resulted in sterile canna hybrids was also noted by Luther Burbank and was considered an advantage. When canna hybrids result in fertile (seed producers) cultivars, the plants energy is focused on seed production, and the blooming process will slow down or completely stop. Gardeners want plants that will flower continuously, and therefore, sterile canna hybrids are more desirable than seed producers. It appears that the crossing of widely divergent canna species will usually produce sterile canna offspring. <br /><br /> If a canna plant is a non-seed producer, it is said to be sterile, however, it may only be considered sterile when examined as a female seed producer, but frequently the pollen (male) from a "so called", sterile male canna species and a backcross onto a fertile female may result in further hybridization with increased hybrid vigor. Those canna offspring may be either seed producers or non-seed producers. This fact was well understood and applied by Luther Burbank who introduced vast improvements in American horticultural crops of flowers, fruits, grains, and vegetables. <br /><br /> Luther Burbank noted in his book, Flowers, Vol VIII, page 41, one of his eight volumes of horticultural writings: "Just now white cannas of very good quality are appearing and every desirable quality in plant and quality are being brought forth." <br /><br /> It was reported on page 33 that Burbank's hybrid canna "Tarrytown" won the grand gold medal, at the Pan-American Exposition in Buffalo, N.Y. as the best canna exhibited at the time. <br /><br /> This canna had a special feature of flowers that dropped to the ground unlike "many canna lilies that tended to hold their blossoms, thus having an untidy appearance." Burbank crossed the Crozy canna, a large flower with varying color (yellow and orange) with Canna flaccida, a native yellow canna flower with large flowers that are not long lasting. <br /><br /> This native Canna flaccida was discovered growing by William Bartram in 1773, the early American explorer and writer, growing in salt water ditches near Fort Frederica, Ga. On the island on St. Simon's as reported on page 153 of his book, Travels, "What can equal the rich golden flowers of the Canna lutea," today identified and renamed, Canna flaccida, "which ornaments the banks of yon serpentine rivulet, meandering, over the meadows?" Canna flaccida still flourishes in salt water ditches there and on the black banks river near the Cloister Hotel at Sea Island, Ga., where it grows near the water beautifully, being used as an ornamental in many yards as a marsh garden plant. These native plants appear to have no insect or disease problems. Mature seeds from the golden-yellow flowers fall into the water and float downstream to establish new canna colonies. Canna flaccida will grow in ordinary gardens if adequate water in available. <br /><br /> William Bartram also reported in Travels, page 424, that he found a native Indian canna with small scarlet flowers that grow up to 9 feet in height, then identified as Canna indica. Many southern gardens today still grow this vigorous clumping canna giant as a privacy hedge. Seed pods are many and this canna is readily crossed with the pollen of hybrid non-seed producing cannas. This plant appears to have a high resistance to bugs and disease. Several reports of Canna indica in the literature do not appear to be the same Canna indica, as described by Bartram in 1773 growing at Mobile, Alabama. An excellent drawing of Canna indica (wild Indian canna) is located on page 218 of Bartram's, Travels. Luther Burbank does not report whether he used Canna indica in his hybridizations, as he had used Canna flaccida (Canna lutea) in his hybridization of the gold medal prize winner at the Pan American Exhibition in Buffalo, N.Y. In 1901, that he called, "Tarrytown," was judged the best canna shown. <br /><br /> Thousands of canna cultivars have been introduced into the world of gardening. It has been quite easy to produce new canna cultivars, for instance, if a dusting of pollen is placed on the female part of a canna flower a seed pod can be formed with several seed, and if multiple flowers of canna plants are pollinated, multiple seed pods can be formed. Any gardener can perform this simple procedure of seed production. <br /><br /> For many years gardeners allowed the canna seed to dry, turning black, and a hard shell eventually developed that would prevent the seed from germinating into a plant unless an iron file had been used to open a hole in the round seed about the size of an olive seed to initiate germination. If a file was not used it might take two years for the canna seed to sprout normally. A technique has been developed that avoids the cumbersome seed germination procedures of the past. The seed pods are harvested as soon as the outside green cells begin to change color to yellowish. The seed inside will vary in color from green to light brown or beige, and should be soaked overnight in a solution of fungicide to control rot. The seed then can be placed in a small cloth bag and allowed to be drenched in running water for 12 hours. If these seed are then soaked for another hour in fungicide and are placed in a flat pan at a sunny warm location they will begin sprouting in a few days. As soon as the seed sprout they may be individually transferred into separate containers to grow, and after the first leaves appear a mixture of miracle-grow will rapidly mature the sprouting seed into flowering plants. TyTy has been successful in growing flowering canna plants only 60 days after sprouting the seed. It is very interesting that when leaved canna plants are crossed, about 25% of the seed will grow into red leaf canna hybrids. This red leaf color is apparent only a few days after sprouting. <br /><br /> It is of great interest to the gardening public that new canna cultivars should feature flowers that fall to the ground after a day or two to be replaced by fresh opening blooms that leave the plant with a fresh appearance, otherwise, withered brown flowers are unpleasant to the eyes of most gardeners. Amateur hybridizers should also keep in mind that most gardeners do not want to experience a canna that must be continuously cared for or nursed, requiring spraying, constant watering or dead-heading. <br /><br /> The popularity of canna lilies has been apparent from the huge plantings at the U.S. Capitol and The White House grounds; serous plantings at Disney in California and Orlando, Fl, and extensive landscapes along U.S. Interstate highways and in city boulevards and parks.   <bio>Patrick A. Music lovers' are turning their attention to the past, reverting back to <a href="http://www.artwoo.com/tag/vinyl+record" rel="tag">vinyl record</a>s.<br /><br /><br /><br /> For some it is an <a href="http://www.artwoo.com/tag/enjoyable+hobby" rel="tag">enjoyable hobby</a>, recapturing a part of their youth. Yet for others, collecting vinyl records is a passion, as they scour the online web sites, record conventions, garage and rummage sales and small resale shops to secure their favorite records and add to their collecting collage of vinyl. <br /><br /> However, if you were to ask them why records are so appealing most will tell you it is the sound of vinyl that is the most enjoyable. They may be tired of the sterile <a href="http://www.artwoo.com/tag/music+reproduction" rel="tag">music reproduction</a> of a cd or digitized download. You will hear the term, "vinyl just has a warmer sound" or that they love the experience, an almost ritualistic pattern that is involved in playing a vinyl record. <br /><br /> But I think the best way to describe the sound that vinyl exudes is an analogy I like to use. A vinyl record is like a "fingerprint" of the recorded music. There is a clearness and clarity to the music, and the sound is actually captured and etched into the grooves of the records. <a href="http://www.artwoo.com/tag/conversely" rel="tag">Conversely</a>, digital sound is like a copy of that "fingerprint" and something is lost when the music is compressed into 1's and 0's, or what I refer to as "binary sound." <br /><br /> Now, this phenomenon may be satisfactory to some and the CD certainly has its place as a musical format. In our throw away society, CD's offer convenience and a method to play music that the vinyl record cannot offer. But "binary sound," to me, is just too clear- you are missing important elements of the recorded sound, sounds that the artist who recorded the music wanted you to hear. <br /><br /> In our <a href="http://www.artwoo.com/tag/digitized+world" rel="tag">digitized world</a> there are compelling arguments for whatever sound <a href="http://www.artwoo.com/tag/reproduction+format" rel="tag">reproduction format</a> a person chooses.
