    <content:encoded><![CDATA[ The link between rheumatoid arthritis (RA), a chronic, systemic, inflammatory, autoimmune disease and the incidence of cancer is still murky. Multiple epidemiologic studies have shown that hematopoietic (blood), lung, and skin cancers are increased in incidence in RA patients, while breast and <a href="http://www.artwoo.com/tag/colon+cancers" rel="tag">colon cancers</a> are decreased. <br /><br /> Whether these cancer rates are caused by RA itself, the immunosuppressive chemotherapeutic agents used to treat RA, or by newer biologic therapies is an issue of ongoing investigation. <br /><br /> Conflicting findings particularly exist over the risk of malignancy related to the use of <a href="http://www.artwoo.com/tag/tumor+necrosis+factor+alpha" rel="tag"><a href="http://www.artwoo.com/tag/tumor+necrosis+factor" rel="tag">tumor necrosis factor</a> alpha</a> (TNFand#945;) blockers, biologic therapies shown to be extremely effective at controlling the progression of RA in patients who do not respond to traditional disease-modifying anti-rheumatic drugs (DMARDs). <br /><br /> A recent study again examined the risk of cancer among biologic-treated RA patients. The researchers examined the extensive databases of both the National Data Bank for Rheumatic Diseases as well as the US <a href="http://www.artwoo.com/tag/national+cancer+institute" rel="tag">National Cancer Institute</a> SEER (Surveillance, Epidemiology, and End-Results). <br /><br /> The scientists studied the incidence of cancer in 13,001 RA patients, over 49,000 years. Nearly half of these patients, 49 percent, had a history of exposure to anti-TNFand#945; drugs. <br /><br /> They found that biologic treatment of RA increases a patient's risk of skin cancers, including melanoma, but not any other specific cancers such as lung, liver, brain, or bone cancers. And there was no increased incidence of other malignancies such as Hodgkin's disease, non-Hodgkin's lymphoma or leukemia. <br /><br /> Among the study population, the investigators found 623 cases of skin cancer and 537 cases of other cancers. They then determined the effect of biologic drug use on cancer occurrence. As an estimate of the relative risk of developing different types of cancer, the authors calculated the odds ratio for every cancer affecting the subjects, performing statistical techniques to reduce the effect of variations in treatment duration. <br /><br /> They also controlled for the variables of gender, smoking history, education level, disease severity, and baseline use of prednisone. In addition to assessing the risk of various cancers associated with biologic treatment in general, the researchers extended the analyses to individual TNFand#945; blockers, etanercept (Enbrel) and infliximab (Remicade). <br /><br /> They found anti-TNFand#945; therapy was linked to an increased risk of skin cancers, including melanoma. The odds ratio for developing melanoma was 2.3. Biologic use had no impact on any other type of cancer. The overall risk for all malignancies was 1.0. (A measure of 1.0 indicates that there is no overall risk for cancers in general, while the risk of 2.3 for melanoma indicates that a given person is 2.3 times more likely to develop a melanoma if they are taking a biologic than if they weren't). <br /><br /> The authors stated, "Although our data do not show associations between malignancy and biologic therapy, except for skin cancers, the mean and median exposure to biologics was only 3.0 years. IIt is possible that with increasing time of follow up or of exposure, the association between malignancy and biologic therapy would become stronger. However, true associations are regularly seen within this time frame." <br /><br /> Despite its limitations, this study offers some reassurance to RA patients. The study validates the fact that for cancers in general, including lymphoma, that there is no significant increased risk for biologic users. Melanoma is caused by the uncontrolled growth of <a href="http://www.artwoo.com/tag/pigment+cells" rel="tag">pigment cells</a> known as melanocytes. The only cure for melanoma is to surgically remove the tumor before it grows to a size of 1mm. There are close to 160,000 new cases of melanoma worldwide each and every year and according to the <a href="http://www.artwoo.com/tag/world+health+organization" rel="tag">World Health Organization</a> there are approximately 48,000 deaths related to melanoma each year as well. 75 percent of all deaths from skin cancer are associated to <a href="http://www.artwoo.com/tag/malignant+melanoma" rel="tag">malignant melanoma</a>. Treatment for melanoma includes the aforementioned surgery, chemotherapy, immunotherapy and even radiation therapy. It is recommended by doctors that anyone who has a family history of melanoma should see a dermatologist at least once a year to make sure they are not developing melanoma themselves.<br><br>The first case of melanoma did not come until the 1960s; at least the oldest known case was discovered until the 1960s. Scientists were working on a group of mummies when they noticed deformities in their skin. It was determined that these mummies did have some type of malignant melanoma. The mummies dated back at least 2,400 years. The first operation related to melanoma occurred in 1787 by a man named John Hunter. The tumor that was removed from the skin was preserved in the Hunterian Museum of the Royal College of Surgeons of England. The mass was not examined until the year of 1968. Upon examination and testing it was concluded that the mass was in fact a form of metastatic melanoma.<br><br>There is a mnemonic device to best remember the signs and <a href="http://www.artwoo.com/tag/symptoms+of+melanoma" rel="tag">symptoms of melanoma</a>. They are superficial spreading melanoma, nodular melanoma, acral lentiginous melanoma, lentigo maligna melanoma, clear cell sarcoma, mucosal melanoma and uveal melanoma. As with all types of cancers, especially malignant ones, there are four stages of the disease that melanoma can be active in. The stages are Stage I, Stage II, Stage III and Stage IV. Each different stage has sublevels that the melanoma can develop in.<br><br>Patients suffering from melanoma can feel a loss of appetite, nausea, vomiting, an uncomfortable feeling and a general weakness throughout the body. The study was headed by Allan <a href="http://www.artwoo.com/tag/conney" rel="tag">Conney</a>, director of the <a href="http://www.artwoo.com/tag/susan+lehman" rel="tag">Susan Lehman</a> Cullman Laboratory for Cancer Research and professor in the school of pharmacy at <a href="http://www.artwoo.com/tag/rutgers+university" rel="tag">Rutgers University</a> in Piscataway, New Jersey. The test used the moisturisers; Dermabase, Dermovan (a wholesale-only product which was discontinued in 2006), Eucerin Original Moisturising Cream, and Vanicream.<br><br>The moisturisers were frequently applied to tester mice with sun-related skin cancer. Allan Conney said that this resulted in faster growing tumors and an increase in the tumors.<br><br>"This was unexpected. We really did not expect to see the tumor-promoting activity of these creams," Conney said.<br><br>Conney and colleagues were eventually intending to use one of these moisturisers, in clinical trials on humans with topical caffeine, which has prevented skin cancer in animal studies.<br><br>"We thought it would be prudent to test Dermabase by itself to see if it had tumor-promoting activity," Conney said. "We did not think it would. But lo and behold, to our surprise we got an increased rate of skin cancer."<br><br>They then conducted further tests on Dermabase and the three other moisturizers, which were also eventually intended for human study. For these new animal studies, the researchers used <a href="http://www.artwoo.com/tag/hairless+mice" rel="tag">hairless mice</a> irradiated with ultraviolet light twice a week for 20 weeks only. These examples eventually developed <a href="http://www.artwoo.com/tag/forms+of+skin+cancer" rel="tag">forms of skin cancer</a>, showing similar traits to humans who are overexposed to sunlight in early life. The moisturisers were then rubbed into the animal's skin for 17 weeks. Resulting in the following, Dermabase increased the total number of tumors by 69%, Dermovan increased the total number of tumors by 95%, Eucerin increased the total number of tumors by 24%, and Vanicream increased the total number of tumors by 58%.<br><br>These results are worrying; if these moisturisers affect mice in this way, then they are possibly also detrimental to humans. Conney said "The answer is, we don't know. Our study raises a red flag and points out the need for epidemiologists to take a look at people who use moisturizing creams. The companies that market these products should take a look at animal models and see if their products promote tumors."<br><br>"This study could definitely be a warning to alert these companies to consider testing moisturizing creams with some sort of assay," said dermatologist Keyvan Nouri, director of dermatologic surgery at the University of Miami Miller School of Medicine and author of the 'Skin Cancer'. He agrees that the companies that make the moisturisers should run further tests on their products.<br><br>Moisturisers are not required to take the same safety tests as medicinal drugs as they are classified as cosmetics. However the results of this test demand the moisturisers to be re-examined, whilst the moisturisers did not cause cancer in the mice they did increase it's growth.<br><br>There is a possibility that the intense radiation damaged the skin of the mice before the moisturizing creams were applied, and this may account for the unusual tumor growth after application. However, cases of skin cancer have increased amongst humans. Skin cancers account for more than half of all cancers, however most skin cancers are curable.<br><br>Conney's research team asked Johnson and Johnson to make them a "custom blend" moisturiser which excluded two ingredients previously linked to skin irritation (sodium lauryl sulfate) and tumor promotion (mineral oil). This moisturiser did not promote skin cancer, yet not all of the products tested used the two excluded ingredients, a problem lies in determining exactly what the link with skin cancer is, also we must take into consideration that mouse and human skin are very different.<br><br>It is important not to instantly condemn moisturisers, as Keyvan Nouri says, they can be very beneficial, "As we get older, our skin gets drier. We need to moisturise, otherwise our skin gets dry and we get eczema, dermatitis, rashes, and so on. I can't recall but once or twice when I have seen someone actually reading the label. The <a href="http://www.artwoo.com/tag/new+york+times" rel="tag">New York Times</a>, July 20 2004, featured an interesting article that explore the mythic links between <a href="http://www.artwoo.com/tag/melanoma+cancer" rel="tag">melanoma cancer</a> and exposure to <a href="http://www.artwoo.com/tag/ultraviolet+rays" rel="tag">ultraviolet rays</a>.<br /><br /><br /><br /> This article differs from others because it was written from the point of a noted <a href="http://www.artwoo.com/tag/dermatologist" rel="tag">dermatologist</a> who cannot find any link between the two. <br /><br /> This dermatologist took a trip to the Middle East, acquired a deep tan, ignoring the hundreds of moles on his body, and not covering his baldhead with a hat, or wearing sunscreen. Three years later, there is still interest in the results of this dermatologist's experiment. <br /><br /> Many reports blame a serious sunburn early in life causes skin cancer later on. While other studies believe there is a small link, and others show none. The fact is, there are so many reports generated by leading research centers, that the masses of accumulated data work against each other to disprove the theory that ultraviolet rays <a href="http://www.artwoo.com/tag/cause+skin+cancer" rel="tag">cause skin cancer</a>. <br /><br /> Some dermatologist now questions if the "epidemic" of melanoma proclaimed by other doctors as a fact. The definition of the cancer has changed over time. Now, doctors are diagnosing cancerous growths that did not carry the label melanoma.  Not all research blames the sun for the increase in <a href="http://www.artwoo.com/tag/skin+cancers" rel="tag">skin cancers</a>. <br /><br /> Dr David G Williams [Alternatives, April 1993; 4: 22] states that the sun might not be the cause, and hiding under sunscreens may increase the chances of developing skin cancer, and increasing the chances of creating problems by interfering with vitamin D synthesis. <br /><br /><br /><br />According to Dr Williams: "Despite what you may have been led to believe, there is not adequate scientific data to support the belief that sunscreens prevent skin cancer (melanomas or basal <a href="http://www.artwoo.com/tag/cell+cancers" rel="tag">cell cancers</a>)." <br /><br /> Some reports, like Dr Williams, blame trans fat oils, like those found in margarine, to offer a more plausible answer to the cancer dilemma than ultra violet rays. <br /><br /> As early as 1950, scientists like Dr Johanna Budwig's published research that demonstrated certain wavelengths of sunlight vibrate at the same frequency as the chemical bonds in unsaturated fatty acids (trans fatty oils are unsaturated). <br /><br /> This, she believes, leads to mutations that become cancerous. <br /><br /> Dr Budwig believed that for a free flow of nutrients to your cells and elimination of toxins, your body needs the supersaturated fatty acids (Omega-3 oils) bound to protein (especially bound to protein if your liver is weak). <br /><br /> The World Heath Organization inadvertently proved Dr. Budwig's report by publishing a paper on rates of cancer around the world. Mexico, which has a large population of people who live in the sun, and has traditionally cooked with lard, had the lowest rate of melanoma cancer -- until they started to industrialize and cook with margarine. <br /><br /> The worsening omega-3:6 ratios, not sunscreen, is the cause of the increases in skin cancers. In 2001, the National Academy of Sciences published a paper showing that the omega 6:3 ratio is the key to cancer prevention. <br /><br /> An Australian study published 10 years ago reveals a 40 percent reduction in melanoma for people who eat omega-3 rich fish.     <content:encoded><![CDATA[ Caffeine, the chemical stimulant in coffee and tea, has been found to lower the risk of <a href="http://www.artwoo.com/tag/skin+cancer" rel="tag">skin cancer</a> in <a href="http://www.artwoo.com/tag/laboratory+mice" rel="tag">laboratory mice</a>. <br /><br /> A study suggests that a skin lotion spiked with caffeine or with another compound found in green tea can reduce by more than half the number of <a href="http://www.artwoo.com/tag/cancer+tumors" rel="tag">cancer tumors</a> on the skin of <a href="http://www.artwoo.com/tag/hairless+mice" rel="tag">hairless mice</a> exposed to brutal levels of ultraviolet radiation, said Dr. Allan Conney, a professor of cancer and leukemia research at Rutgers University in New Brunswick, N.J. <br /><br /> "We had between 50 to 70 percent tumor formation inhibition in the mice that were treated with caffeine or with EGCG (the other chemical compound)," said Conney, senior author of a study appearing this week in the online site of the <a href="http://www.artwoo.com/tag/proceedings+of+the+national+academy+of+sciences" rel="tag">Proceedings of the National Academy of Sciences</a>. <br /><br /> Skin cancer is the most common of all cancers in the United States. The American Academy of Dermatology estimates about a million cases will be diagnosed in the country this year. Among them will be more than 88,000 new cases of melanoma, the disease's deadliest form. Skin cancer generally is curable by cutting, burning or freezing the tumor cells, but untreated it can be deadly. <br /><br /> To test <a href="http://www.artwoo.com/tag/effects+of+caffeine" rel="tag">effects of caffeine</a> on skin cancer, Conney and his colleagues exposed 90 mice to high levels of ultraviolet radiation twice a day for 20 days. They used a strain of animals, called hairless mice, commonly used for skin cancer studies. <br /><br /> After the mice got their UVB doses, the animals were divided into three groups. One group were slathered daily with a solution of acetone and caffeine. Another group received acetone and EGCG. The third group got skin applications of acetone only. Acetone is an organic solution often used on the skin. <br /><br /> At the end of 18 weeks, the three groups of mice were killed, and the level of skin tumor formation was analyzed. <br /><br /> Conney said mice in all three groups developed malignant skin tumors, called <a href="http://www.artwoo.com/tag/squamous+cell+carcinomas" rel="tag">squamous cell carcinomas</a>, but the number of tumors per mouse was reduced by 72 percent in those treated with caffeine and by 66 percent among those treated with EGCG, compared to the controls treated only with acetone. <br /><br /> The treated mice also had fewer nonmalignant, sunlight-related tumors, said Conney. Compared to the control group of mice, the mice treated with caffeine had 44 percent fewer nonmalignant tumors, the EGCG group 55 percent fewer, he said. <br /><br /> Conney said that although both compounds were effective in lower tumor risk, caffeine has an advantage because it is chemically more stable than EGCG. <br /><br /> Unlike sun screen lotions, which protect against skin cancer by preventing the skin from absorbing ultraviolet rays from the sun, the caffeine's cancer protection works in the cells after exposure to the ultraviolet rays. Rays from the sun can cause genetic changes in the skin that can lead to skin cancer. Conney said caffeine apparently blocks this action by causing abnormal cells to kill themselves, a type of programed cell suicide that prevents development of abnormal growths. <br /><br /> "This is not a sunscreening effect," said Conney. "It is a biological effect." <br /><br /> He said the caffeine acts selectively, causing the abnormal cells to die but not affecting the normal cells. <br /><br /> Caffeine, heavily consumed in coffee, tea and some cola drinks, has been shown in other studies to prompt mental alertness in many people. Some studies have suggested caffeine aggravates symptoms of menopause or intensifies the side effects of some antibiotics. Heavy caffeine use has been linked to miscarriage. Some studies also have suggested that some people can become addicted to caffeine and can experience headaches and other symptoms when deprived of their morning coffee or cola. <br /><br /> Dr. Darrell Rigell, a professor of dermatology at New York University and an expert spokesman for the American Academy of Dermatology, said research like Conney's is needed badly because "skin cancer is a major problem. I hope this treatment can prove itself, because there are more skin cancers than all other cancers combined in the U.S." <br /><br /> He said there is a need for a "morning-after" treatment for skin cancer, a therapy that would reduce cancer risk after excessive sun exposure. <br /><br /> Rigell said that although hairless mice are commonly used for such research, "there is really no good animal model for skin cancer. The hairless mouse is the best of a bunch of bad choices" for testing skin cancer compounds in the laboratory. <br /><br /> As a result, he said, "a lot of things that work in mice cannot be extrapolated to humans." <br /><br /> He said other treatments that showed promise in mice have often failed when tried on humans. <br /><br /> Conney said the next step in studying the topical effects of caffeine will be to use the solution on people who are highly susceptible to skin cancer =97 people who have a precancerous condition or who already have had skin cancer.   <bio>JH Reynolds is a coffee enthusiast who travels around the world in search of the best beans and exotic flavors.
