    <content:encoded><![CDATA[ Most people are not familiar with the term "<a href="http://www.artwoo.com/tag/turbinates" rel="tag">turbinates</a>" unless if these complex, critically important structures are removed from the nose. These structures, the primary ones which are similar in size to our <a href="http://www.artwoo.com/tag/fingers" rel="tag">fingers</a>, are sometimes removed or reduced in size in the nose to help open up the airway. Turbinates can become swollen and block the nasal airway because of allergies, non-allergic rhinitis, overuse of intranasal decongestants, or it may be caused by a septal deviation. So doctors trim them down and for good reason. <br /><br /> Remove too much of the turbinates, though, and your life will become a mess. It is sort of like removing your fingers - but far worse. Your breathing and nose become significantly affected. Just like your hand won't be fully functional if you remove your fingers, neither will your nose if you remove the turbinates. These structures are not visible to the eye because of their location in the nasal cavity, so you can't tell if someone's turbinates are present and fully functional or if they have been removed =96 until you look at the CT scan. Because these structures are "hidden," in a sense, removal is not as obvious of a problem as people who have their fingers removed. The end consequence of turbinate removal is there is nothing left to remove from the nose! It is already gone! <br /><br /> The turbinates consist of bone sandwiched between mucous layers, and they are rich in blood vessels and nerve endings. The outer portion is thick with significant <a href="http://www.artwoo.com/tag/mucus" rel="tag">mucus</a>-producing capabilities. The turbinates serve many important functions for the 18,000 <a href="http://www.artwoo.com/tag/liters" rel="tag">liters</a> of air we breathe and 1-2 liters of mucus that goes through our nose and sinuses each day, including: <br /><br /> 1)Directing airflow. The nose directs airflow in an orderly pattern so that air is experienced throughout all regions of the nose. <br /><br /> 2)Providing <a href="http://www.artwoo.com/tag/nasal+airflow" rel="tag">nasal airflow</a> <a href="http://www.artwoo.com/tag/resistance" rel="tag">resistance</a>. The nose provides greater than 50% of resistance in overall airflow to our lungs, ensuring optimal lung functioning. <br /><br /> 3)Containing nerve cells. Laminar airflow strikes the nasal mucosa, which is embedded with trigeminal receptors (nerve receptors that detect airflow motion and temperature) and these nerve cells tell the brain you are breathing. <br /><br /> 4)Humidifying the air we breathe. The turbinates help moisten the air we inspire so it is near 100% humidity by the time it reaches our lungs. <br /><br /> 5)Warming the air we breathe. Turbinates help warm air not only by providing nasal airflow resistance, but also the larger and lower turbinate (the inferior turbinate, which is rich in blood vessels) has significant expanding (to decrease cold air intake) and contracting (to allow more air to enter our nose) capabilities. Specifically, the anterior portion of the inferior and middle turbinates plays a significant role in directing and warming air upon inspiration. <br /><br /> 6)Filtering the air we breathe. The turbinates provide much surface area to allow infectious particles to strike them, entrapping them in mucus, which is swept into the pharynx where it is harmlessly swallowed. <br /><br /> 7)Smelling the air we breathe. The turbinates project a small amount of air, 10% or less, toward the superior turbinates where olfactory bulbs are present that help detect smell. <br /><br /> 8)Swelling. The turbinates swell on one side of the nose while the other side constricts ever 2-4 hours, a process known as the nasal cycle. <br /><br /> Clearly, the primary turbinates are incredibly complex, serve many important functions, and should be treated with utmost respect. It is important before undergoing any nasal or sinus surgery to have an honest, open discussion with your doctor about how the surgery will affect the mucosa of the turbinates.     <content:encoded><![CDATA[Whenever I talk about irons in my golf lessons, students invariably ask which style is best--<a href="http://www.artwoo.com/tag/cavity+back" rel="tag">cavity back</a> or <a href="http://www.artwoo.com/tag/blade" rel="tag">blade</a>. The cavity back has its advantages and its disadvantages. It also has its fans. <br /><br /> Golfers, however, shouldn't choose an iron style because it's popular in the clubhouse, since the style many not fit their game. The key to iron styles, as I've said in my <a href="http://www.artwoo.com/tag/golf+tips" rel="tag">golf tips</a>, is finding what's right for you. <br /><br /> Cavity backs are popular these days--and for good reason. A cavity back has a small amount of metal removed from the back of its clubface, producing a small hole. Removing the metal re-distributes the clubhead's weight around the edges of the clubface, father away from the center of gravity (<a href="http://www.artwoo.com/tag/cog" rel="tag">COG</a>). <br /><br /> Re-positioning the COG creates a much more forgiving iron, with a larger sweet spot along the blade. Thus, a <a href="http://www.artwoo.com/tag/mis" rel="tag">mis</a>-hit with a cavity back is more likely to stay on <a href="http://www.artwoo.com/tag/target" rel="tag">target</a> than a similar shot with a blade. Why? Because the cavity back twists less in a player's hand when the ball is mis-hit. A mis-hit with a cavity back is also more likely to travel farther than with a blade. <br /><br /> Cavity backs are "game improvement'' clubs, offering special features that help golfers play better, like an oversize head. I've talked about these clubs in my golf tips. Players with high and mid <a href="http://www.artwoo.com/tag/golf+handicaps" rel="tag">golf handicaps</a> prefer cavity backs, although some low handicappers and touring pros use them. <br /><br /> Blade irons are not as popular as cavity backs. A blade iron features a solid clubface back, distributing the weight more evenly across the clubface, closer to the clubhead's COG. Thus, a blade has a much smaller sweet spot than a cavity back. A blade is also much less forgiving than a cavity back because it twists more in a player's hands on mis-hits. <br /><br /> Distributing the weight evenly across the clubface, however, creates an iron with better control and more feel. These irons need to be hit nearly perfectly, though, to avoid a bad shot. Thus, it takes a lot of practice and experience to hit these irons well, something I work on in my golf lessons with low handicappers. <br /><br /> The blade iron is known as a more traditional iron because it lacks the cavity back's special game improvement features. Players with low golf handicaps and touring pros prefer the blade style iron because the added control and feel enables them to shape their shots better--a necessity when playing on challenging courses. <br /><br /> Manufacturers make cavity backs and blades in cast and forged versions. The terms "cast" and "forged" refer to the manufacturing process used to form the iron head's shape. <br /><br /> Casting turns the metal from which the iron head is made into a molten liquid, which is then poured into a mold to form the iron head. It's then left to cool. <br /><br /> Forging involves pounding or compressing the metal, in it's solid form, from which the iron head is made into the desired shape. Other machining and drilling steps complete production.  <br /><br /> The manufacturing process has no impact on the iron's capabilities, as I've explained in previous golf tips. If you have two irons, one forged and one cast, of exactly the same shape, with the same center of gravity, same loft, same grip, hitting the same ball, and so on, the shots will travel the same distances 99 percent of the time. And the players won't know which iron head is cast and which forged. <br /><br /> You need to find the iron style that best fits your game, as I point out in my golf instruction. If you're a less experienced golfer, the cavity back is probably a wiser choice, since you're more likely to mis-hit a ball. If you're a more experienced player, then a blade is probably your best choice, since it provides more control and better feel for shaping shots. <br /><br /> The best way of choosing a style that fits your needs is to test it out. Hit a few balls with each style. Pick up a magazine there's ads, the television is full of ads -- these products are heavily marketed and choosing the one that's going to work is an entirely different game. <br /><br /> There are four allergy medication categories -- nasal sprays, decongestants, nasal antihistamines, and oral antihistamines. Let's have a look at all four categories. <br /><br /> Oral antihistamines are by far the most common allergy medication prescribed by doctors. The block the histamines which is the chemical that is responsible for most of your symptoms. A submucous resection procedure is used to either 1) remove part of the <a href="http://www.artwoo.com/tag/turbinate+bone" rel="tag">turbinate bone</a> while not tampering with overlying flaps of mucosa, or 2) remove vascular space tissue, which might be enlarged in patients with drug-induced rhinitis or <a href="http://www.artwoo.com/tag/sleep+apnea" rel="tag">sleep apnea</a>. <br /><br /> 2) Submucous resection with outfracture. In addition to submucous resection, this procedure includes fracturing (breaking) the turbinate bone and then applying pressure on the bone to move it to the side or lateral wall of the nose. <br /><br /> 3) Electrocautery - Use of electric currents with heat to reduce the size of the turbinate.<br /><br /><br /><br /> 4) Radiofrequency =96 Use of heat to induce submucosal tissue destruction. <br /><br /> 5) Total and partial turbinectomy =96 Removal of an entire turbinate (s) or two-thirds of the turbinates.<br /><br /><br /><br /> 6) Laser Surgery =96 Shooting of a laser beam that is absorbed by the turbinate tissue, causing it to evaporate.<br /><br /><br /><br /> Clearly, a thorough discussion of what precisely will be done and how it will affect the mucosa of the turbinates must take place before any type of nasal surgery.
